  5. Principal Shippers: List the names of your major regular shippers.
  6. Receipts/Mileage/Units: Provide operational data of your business.
  7. Commodities Transported: Describe the types of your business hauls, including average truckload, description of each commodity, and percentage of total revenue that each represents.
  8. Equipment: Specify the number of vehicles, types, radius of operation, and ownership status (owned, non-owned, leased, or long-term leased).
  9. General Information: Answer each question about your operations in “Yes or No”. Explain “Yes” answers in the blank space provided.
  10. Remarks: Explain additional information not captured elsewhere. Or attach the Acord 101 if you need more space.
